Animal Welfare 

• Request to Obtain Grooming, Pet Shop or Kennel Permit

  • Submit this form to the Animal Welfare division at the Karen Kay Mullendore Municipal Animal Complex at 1710 S. Main St.

  • After an Animal Welfare officer inspects the business and/or kennel, you will receive a second form to submit to the Stillwater Municipal Building at 723 S. Lewis St. to purchase the permit.
